Instructions for filling out the work registration application form:
Required fields:
1. Title of work: Fill in the title of the work for which you are applying for registration, and make it as short as possible. The title of the work in all application materials must be consistent.
2. Work category: Select based on the content of the work fill in. (For computer software, please apply directly to the China Copyright Protection Center for registration)
3. Copyright owner: For personal works, fill in your name. For unit works, fill in the name of the unit and keep it consistent with the identity certificate. In the signature column, you can choose to fill in your name, alias, or no signature. The signature must be consistent with the work.
4. Author's name: For individual works, fill in the individual's real name and circumstances. For unit works, if the signature right belongs to an individual, fill in the personal circumstances; if the signature right belongs to an organization, fill in the organization. In the signature column of the work, fill in the signature on the work.
5. Nature of work creation: Check according to the form of creation. (Except for original works, other derivative works such as original works within the copyright protection period should submit a licensing agreement and a copy of the original author’s identity certificate)
6. Creation or production completion date: Fill in the year, month and day when the work was completed.
7. Place where creation or production was completed: fill in the place where the work was completed.
8. Publication status: Check according to the situation. If it has been published, fill in the first publication date and the country and city of publication.
9. Description of rights status: Check and explain according to the situation.
10. Retention sample: Fill in based on submitted materials.
